Osteoarthritis (OA) is the most common type of arthritis. High prevalence, especially in old age and the resulting disability, has been the main cause of disability in old age. Due to the aging population of Western countries and the increasing prevalence of obesity (which is a major risk factor), osteoarthritis is on the rise, so that by 2020 its prevalence in the United States will increase by 100-66%.
The patient's complaints are usually pain, joint stiffness, morning joint stiffness, and limited mobility. Therapeutic goals for osteoarthritis are pain relief and minimization of physical function. Such as topical and oral analgesics) and surgical procedures are used.
Due to the dangerous side effects of common painkillers such as nonsteroidal anti-inflammatory drugs (NSAIDs) that may increase the risk of heart attack and stroke as well as ulcers and gastrointestinal bleeding, the use of effective and safe herbal medicines is becoming more and more popular.
Among the herbs effective in relieving pain and inflammation are mint and celery, and many studies have proven their effectiveness. Menthol is one of the active ingredients in mint that its counter-irritant and local pain effects are fully accepted.

When peppermint or menthol essential oils are rubbed on the skin, they simultaneously stimulate the sensory nerves to perceive the cold and the sensory nerves to suppress the sensation of pain.
Due to the effect of the initial cooling, there is a period of heating of the position. In a randomized, double-blind, placebo-controlled crossover trial, topical application of peppermint essential oil on the forehead and temporal lobe statistically reduced muscle tension reported by patients and the electromyographic activity of the temporal muscles.
In addition, topical application of peppermint essential oil has reduced pain sensitivity. According to the results of animal studies, menthol has analgesic effects by activating the endogenous opioid system or, to some extent, by local anesthetic without anti-inflammatory properties.
Peppermint essential oil also inhibits muscle contractions caused by serotonin and substance P. Based on the results of animal studies; Celery essential oil has anti-inflammatory, antispasmodic and sedative effects.

Peppermint essential oil has anti-inflammatory and antispasmodic effects. In the study of analgesic and anti-inflammatory effects of peppermint essential oil and celery essential oil and their mixtures in rats and mice, it was found that in carrageenan test; Peppermint essential oil, celery essential oil and a mixture of these two essential oils have significant local anti-inflammatory effects.
Peppermint essential oil and a mixture of peppermint-celery essential oils significantly inhibited pain in the first phase of the formalin test, which is a neurogenic pain and is caused by direct nerve stimulation. Also, peppermint essential oil, celery essential oil and a mixture of these two essential oils showed significant inhibitory effects on pain in the second phase of formalin test, which is an inflammatory pain.
The active ingredient in Siam lotion (peppermint essential oil and celery essential oil) inhibits prostaglandins and other inflammatory mediators, so it can relieve inflammation and pain.
In one study, mice were divided into experimental group (n = 12) or control group (n = 12). Tail-flick (T.F.L.) method was used to determine the analgesic effects of Siam lotion.
At an intensity of 40, 1 ml of siam lotion was applied on the tail of each mouse in the experimental group. Thirty minutes later, the mouse was placed in a T.F.L. machine and the device recorded pain endurance time in each mouse.

The mean time was 35.5 seconds for the experimental group and 3.2 seconds for the control group.
This study showed that soy lotion increases the duration of pain resistance (p <0.05) Karun; It is a monoterpene ketone that is the main active ingredient in mint plants such as Mentha spicata.
In one study, the analgesic effect of this substance was investigated using various experimental pain models and the involvement of these effects in neuronal irritability induced by other monoterpenes.
The results of this study showed that Karun has an analgesic effect with a decrease in peripheral neuronal irritability.
Karun; In various tests, it reduces the pain response without the intervention of the opioid system. This effect may be related to its ability to reduce the amplitude of the combined action potential.
In a double-blind, placebo-controlled study, 102 patients with osteoarthritis of the knee were randomly divided into three groups: Mint Lotion, Mint-Celery Lotion (Siam Barige Topical Lotion), and Placebo Lotion to apply topical lotion four times a day. .

Re-examination was performed 4, 3, 2, 1 week after the initial examination and the severity of patients' pain in the positions of walking on a flat surface, climbing stairs, descending stairs, sleeping, sitting and climbing downhill was evaluated and the information obtained with Statistical tests were analyzed.
Based on the results of this study; 2 and 4 weeks after starting treatment; The effect of mint lotion and Siam Barige topical lotion in reducing pain intensity when walking on a flat surface was significantly different from placebo (p <0.01). Siam Barige topical lotion also had relatively better effects on pain intensity when walking on a flat surface and sitting (although not statistically significant) than mint lotion.
After 2 weeks of treatment, the severity of sleeping pain was significantly reduced in the Siam Barij topical lotion group compared to the mint lotion group and the placebo group (p <0.01).
